While the message hints at deeper meaning—mentioning astrological signs like Cancer, Pisces, Cetus, and Delphinus—these do not affect your experience, since some are not even real-world signs.

The real outcome of entering your birthday is a special in-game celebration. If you log into Death Stranding 2 on your chosen birthday and visit your private room, you’ll get a unique cutscene, some bonus Likes, a stamina-boosting slice of pizza, and Heartman’s Star-Shaped Glasses. These glasses are a special cosmetic item you can’t get any other way.

If you want these rewards early, you can enter any date as your birthday—even a day soon after you start playing. This lets you see the cutscene and unlock the glasses right away. Otherwise, if you miss your birthday, you won’t lose out on any core gameplay features.
